All students must meet the NYS Immunization Requirements.

Physical Exam for all new entrants and students in grades K, 2, and 4:
In the elementary schools a physical exam is required for all new entrants and students in Kindergarten, 2nd and 4th grade. This exam must be performed by a United States physician within the past 12 months. Please download the Health Appraisal Form for your physician to complete. This form should be submitted within 30 days of the start of school in September. Your child MAY NOT participate in physical education without this on file.
Vision and Hearing screenings are required during the school year in Kindergarten and grades 1, 2, 3, and 5.
Scoliosis screening grade 5:
A scoliosis screening must be performed in grade 5. A copy of the child's most recent physical exam with the scoliosis results noted is acceptable (click on the link for the Health Appraisal Form above).
