School Beginning Reminders
Please return the Health Appraisal Form, and most recent immunization record, if you have not already done so.
Students in grades K, 2 and 4 are required to have a physical on file prior to entry into that grade. If your child has had a physical, please return the Health Appraisal Form to the Health Office. If not, please make an appointment with your pediatrician.
We highly recommend that you submit both your child's annual exam and immunization record, each and every year. This allows your child to remain in school, participate in physical education, fulfill necessary mandated screenings, and meet immunization requirements.
